Package Policies

The Answers You Need

  • All Packages: Pre-paid packages of 4-6 or less must be used within (6) months of purchase or 180 days.
    Pre-paid packages of 12 must be used within (12) months of purchase or 365 days.
    CareCredit Packages of 3 must be used within (3) months of purchase or 90 days.
    Series Treatment Packages must be used within (2) months of purchase or 60 days.

  • Pre-paid packages are only good for the services purchased and cannot be combined or redeemed for any other goods or services.

  • Packages are non-transferable and non-refundable

  • Any unused services do not roll over. 

  • If you no-show to a scheduled appointment you will be charged 100% of the scheduled services at full price or one of your package services will be redeemed as payment.
